2. Healthy Living

A Registered Nurse will work with you to support lifestyle changes and learn skills that will improve overall health.

Healthy Measures is a 6-week workshop that runs every few months. It focuses on having "vitality" by practicing healthy eating and physical activity.

3. Seniors Health

A Registered Nurse will work with you to promote health and independence through education, consultation, referral and memory assessments.

4. Chronic Disease Management

A Registered Nurse will work with you to manage chronic diseases including vascular disease, diabetes, arthritis and breathing disorders.

Many medical practices throughout the world have moved towards INR management programs overseen by pharmacists and registered nurses. At the Haliburton Highlands Family Health Team and the Family Medical Centre, our RN's will monitor and make necessary and timely medication changes according to client's INR.

Building Bridges to Better Health

This program is delivered in partnership with St. Elizabeth Healthcare. It is a series of 2.5 hour workshops spread over six weeks where people with different chronic diseases meet to discuss and learn the skills needed to deal with their condition on a day-to-day basis in an effort to maintain and/or increase life's activities and enjoyment. Course will be held Thursdays, May 7- June 11, 2009 - 1:00-3:30 pm at the Haliburton Highlands Family Health Team office located at the Medical Centre in Haliburton.

Cardiac Education

Six weekly educational sessions to assist clients after a cardiac event/intervention. Focuses on education about heart health, medications, nutrition, risk factors, exercise and stress.

Haliburton County Stroke Support Group

This support group meets every 3rd Thursday of the month from 10:00-12:00 at the Fireside Lounge at Hyland Crest in Minden. This support group is for stroke survivors and/or caregivers.

Living WIth Stroke

Eight weekly interactive education sessions for clients living with a mild to moderate stroke plus their caregiver. Supported by the Heart and Stroke Foundation of Canada. Course begins Mondays, March 23 to May 11, 2009 - 10:00-12:00pm at the Haliburton Highlands Family Health Team office located at the Medical Centre in Haliburton.

Vascular Care Educational Program

Three individual sessions focusing on understanding vascular disease, healthy eating and physical activity.

5. Healthy Kids

A Registered Nurse will support families in understanding and maintaining their children's health through education, consultation and will facilitate referral to other community services.

6. Pharmacy Services

A pharmacist will work with you to assist and teach you about your prescribed medications.

7. Social Work Services

A Social Worker will work with you to support and link you to community services.

8. Nutrition Counselling

A Registered Dietitian will work with you to understand and practice health eating habits for you and your family.

9. Chiropody Services

A Chiropodist will work with you to treat foot problems.

Access Centre Case Management

A Case Manager with the Central East Community Care Access Centre is readily available to arrange client homecare services if needed.
